Application Form

Application for membership to NWONSCIA can be made by downloading the form and submitting to the group's membership coordinator.

Basic membership entitles you to vote at local and national meetings. You will receive our monthly chapter newsletter, "The Spinal Rap" and be recognized as a member of the Northwest Ohio Chapter and the National Spinal Cord Injury Association (NSCIA) www.spinalcord.org . Your active input and participation with the various events is highly encouraged.

Patron membership includes the benefits of the basic membership, as well as, advertisement noted through recognition in the monthly newsletter, "The Spinal Rap", and your business/organization/company's generous support will be noted on our official web sites, www.nwonscia.org and www.spinalrap.org with a link to your own site (if available).
